Dr. Joshua Liao: Value-based Care Transformer Winner for the 2025 Bold Awards

What distinguishes Dr. Liao is not only his rigorous expertise in evaluating the impact of value-based policies, but also his pragmatic contributions to the nation's most influential value-based care advisories. He stands among only several people in history to serve on both the Medicare Payment Advisory Commission, an independent legislative branch agency advising Congress on Medicare policy and value-based care models, and the Physician-Focused Payment Model Technical Advisory Committee, a group of national experts advising the U.S. Department of Health and Human Services on innovations in value-based payment. He is the only person to ever serve simultaneously on both.

These roles build on a long legacy of national service, including advising state Medicaid agencies, working with the Centers for Medicare & Medicaid Services, and serving on key committees or advisories for national groups, including the American College of Physicians and the American Academy of Family Physicians.

Transforming Care Through Evidence, Evaluation and Bridging Leadership
Dr. Liao's intellectual range and bridging interests began in his training. He earned dual bachelor's degrees in English Literature and Biochemistry & Cell biology from Rice University before receiving his medical degree from Baylor College of Medicine and completing internal medicine training at Brigham and Women's Hospital, where he was a clinical fellow in medicine at Harvard Medical School. He received advanced policy training at the University of Pennsylvania, where he served for a decade as fellow of the Leonard Davis Institute of Health Economics in the Wharton School.

Dr. Liao leveraged this diverse training and perspective into a career that embodies an uncommon blend of clinical practice, policy expertise, executive leadership, research rigor, and commitment to public service.

Beyond his national policy leadership, Dr. Liao has shaped value-based care at the state and health system levels. In Washington, he founded a nationally leading policy evaluation unit that formally partnered with the Health Care Authority, leading evaluations that informed decisions on telemedicine expansion, primary care transformation, and other issues for the state's largest purchaser of health care. He continued this work in Texas as a Distinguished Chair and Tenured Professor of Internal Medicine, where he led the Program on Policy Evaluation and Learning to achieve the mission of using data and insight to drive value-based payment and care delivery.

Over the same period, he also led actionable health system transformation, serving in multiple roles for large integrated delivery systems-including enterprise-wide medical director of payment strategy, value-based care and contracting; associate chair for health systems; and chief of a large general internal medicine group. Currently, he serves as a physician executive at Ascension, where he leads national strategy and implementation of clinical transformation programs that use care delivery and technology solutions to drive patient and clinician engagement and improve population outcomes.

The influence of Dr. Liao's work is reflected in several ways, including strong business unit growth and performance, as well as an academic record that boasts over 330 publications (including more than 260 in peer-reviewed journals), nearly 100 national and international presentations, and over $15 million in funding.

Collectively, his contributions have shaped the field's understanding of how value-based programs influence clinicians, health systems, patients, and communities. Importantly, he has done so using a unique ability to connect diverse viewpoints. As his nominator wrote, "He is a 'bridging leader'-someone who can engage stakeholders with a wide range of perspectives while remaining steadfastly committed to the public good."

Despite his achievements, Dr. Liao's influence is not defined by positions, titles, or committees; it is defined by a bold vision for a more equitable, effective, and people-centered health system, aligned under a north star of improving health by improving health systems.

Ultimately, his career has demonstrated that transformation is possible when evidence, policy, innovation, and collaboration converge with persistence, strategy, and vision. His work has strengthened local, regional, and national programs, empowering the policy and practice communities to build a more sustainable future.
